Frequently cannot attach items on a new EVA, and Kerbal jet packs failing

Frequently (possibly always) when my Kerbal goes on EVA and then takes a part, he cannot attach it to anything. Right clicking the part shows the Kerbal's fuel (and if I'm on the ground the flag prompt) as if I had right clicked him. Hitting "H" shows the text for how to place the item, but when I use the mouse to place the item I don't get the ghost item.
If I go back to the Space Center, and then use the Missions view to go back to my Kerbal, he can place the item the next time I try. However then frequently (possibly always) his jetpacks stop working and if he's floating, he's essentially stuck in space. Going back to KSC and back doesn't fix that every time, but eventually he'll unlock and be able to use his jets again. I haven't found a consistent way to bring him back to life, though. It just seems to be a matter of time. If he's on a ladder when he places the item, he has no ill effects even if he later leaves that ladder.
Once I get through those 2 problems, that Kerbal can take and place items all day long with no issues. Until he enters a capsule and comes back out, and the whole cycle starts anew.
