PVG Stuck on "Awaiting Liftoff" (2.12.0)

If I initialize PVG, it is stuck on "Awaiting Liftoff" with the error IndexOutOfRangeException: Index was outside the bounds of the array.
Resetting and reinstalling MechJeb does not work.
