Mechjeb 2.1 Docking Autopilot Misses All the Time

Latest release of Mechjeb docking autopilot misses by about .5 meters every time. Spacecraft unable to dock when this far off center.
- Have you selected the docking ports explicitly on both sides (control from here; set as target)?
- Could this be issued due to a wrong attitude adjustment?
Could you please provide further information.
