On the step 'warping to start of braking burn' the auto-warp is just a little too aggressive. I find that my ship almost always gets sent into a roll as the warp starts, then it stops, adjusts the orientation and warps again to the same result. If I turn off the auto warp and give the vessel an extra second to stabilize before turning on auto warp then it works fine.
