Please explain your issue. How do you trigger it?
Alright, So I've had this issue for a long time now.
Sometimes whenever myself or my players relog or come onto the server, Their prefix is gone/missing.
I've also realized this whenever I reload the server when I do /reload. The person who does the command loses the prefix. Is there any way to fix this?

No errors, It's always been there.

It also happens when you first get onto the server then walk or talk.
