What kind of potion/item are you using when this error occurs?

Not quite sure, as it happened to a random player and I just managed to spot the error while going through some logs. Don't have much info on this.

Could it be caused by a Witch throwing potions? I was just spectating a player that was in a fight with a witch and saw the same error after a potion hit him

This seems to be related to issue #220 . I'll have a look at it now.
