Your permissions.yml file is so incorrect, it shouldn't even load and PEX shouldn't function at all.
The error message in your "Detailed Infromation" section means there's no "default" group into which all users are placed when they first join the server. To give your server a "default" group, you have to set the options: setting default: true for one group.
See this example to better understand what a permissions.yml file should look like.
It is also highly recommended that you update your server. Minecraft 1.8.8 is very outdated; Minecraft 1.11.2 is the current version.
The version of PermissionsEx you're trying to use— version 1.20 —is for Minecraft 1.6.2, which is more than 4 years old and will not function correctly on your server.

The change in battle mechanics made PvP more about skill, and less about who can click a button faster.
Instead of restricting yourself to pre-battle mechanic update, why not just look for modern mods and plugins that make the battle mechanics more functional. You and your players are missing out on a lot of content by using an obsolete version.
"Better Combat Mod" works alongside the changed battle mechanics and further enhances the skill in PvP. It will get you to 1.11.x
There are several weapon/armour/item mods/plugins on Curse/CurseForge, as well as several other official communities that can point you in the direction of resources for a kit pvp server
(It is strongly recommended to stay clear of sites like 9minecraft and minecraftsix. They are not known for redirecting download links to their own servers, and you're not guaranteed to get a clean file)

If you do stay with Minecraft 1.8.8, you can still use the example file I linked above. It's the correct format starting at Minecraft 1.7.9, minus the note about every group requiring an inheritance: section.
