    I tested this bug with other books, copied the book in its entirety and even tested it with copies of copies, but this only happened with 1 book. I am worried it might happen with other books and simply goes unreported..

This looks like a spigot bug to me at first glance. Try it with a newer version of spigot.
I even think to remember this issue from the past. I think the spigot team has already fixed it in newer versions.
