Add custom item model support

Why?

In 1.21.4+ you can set/get the item model of an item stack with ItemMeta#getItemModel()

Other

There is currently custom model data, which is different from the item model, so I think the docs and syntaxes will need to be cleaned up to make it simple to understand that they're two different types.
