You probably already tried this, but just to be sure, did you try a locate command?
/locate structure betternether:pyramid.

There are multiple pyramid types that are randomly chosen, so if you can find one of them, the others should also generate.

i did use that command, i went 10k blocks in multiple directions and multiple worlds but did not find it.

This appears to be a combat issue, as in general this seems to work. So I need to know which mods you have installed. Also a log of a session where you used the locate command might also help.

Here is the log of the world i play on with the mods. I created a new profile on cruseforge
and found the big pyramid after updating the BetterNether mod to 8.1.1
