[Suggestion]: add whether or not block swap works in game vs exclusively on world load to mod page description

What happened?
Due to it not being mentioned anywhere (feel free to correct me if it is), it would be helpful if it was stated if swapping can happen with newly generated chunks while in game, or if it is only on opening a world. currently the mod description states that it reloads the config on world open, but if it does not replace blocks in-game, it would be helpful to state this, since currently the description could be misleading. If it does replace blocks in-game, i may have set up the config incorrectly.
