Issue description

There are critical design flaws in how this code works. At least two gamebreaking exploits have been found due to this, and more probably exist. We need to redo it, possibly at the cost of accuracy.

Please coordinate on the task and with me over #vazkii on IRC, we'll probably want the fixes reviewed heavily.
