Hello,
I've recently found out Carpet Mod was the reason my server crashed every time a piston retracted, leaving behind a ghost block.
When the ghost block is set to air, the piston extends, and when it retracts, the server crashes again, giving everyone the "Disconnected" screen.

Your crash is caused by fixedwaterlogging.

Are you on 1.4.118? A crash involving pistons (and MBE) was fixed on 1.4.119. I just published it on modrinth, previously it was on github and curseforge (modrinth publish isn't automated yet).

If it's not that, the crash report would be helpful.
