Chance Icosahedron Trigger random reward with tier 3 pendant in inventory crash.

I placed a Icosahedron. Inspected with the lens thing, and then it was 100 / 100 with Tier 3 pendant in inventory. It crashed game.

Seems it did give me an Enchantment table in my inventory and lets me reload back into the game.

Minecraft, Forge, and ChanceCubes versions?

Fixed the link and I think I know where the error is, but I need a little more info.
