The Issue:

The problem is that, when you get the item of destiny reward, and then get an experience bottle (mine had fortune 1, smite 5, fortune 1) and throw it, it will delete the chunk you're in)
important to note this was on a server

I'm seeing nothing in your logs to point back to Chance Cubes. Given the interaction you provided above is all vanilla. There's nothing I can really do.
