items in tool rack vanish when tool rack is broken

Describe the Issue
Playing on a singleplayer world. An iron hoe was on a tool rack. I broke the tool rack without removing the hoe from it. The hoe vanished into thin air. I placed the tool rack on the wall and placed a spider eye in it. When I broke the tool rack again, the spider eye vanished. I couldn't find either of them under the floor or outside my house or elsewhere.
The issue still occurred when I tested it using a feather with optifine turned off.
