server crashing when one specific player dies in the nether to various different mobs

tried reproducing on my end by doing what the player is doing but no luck reproducing.

He was playing alone on server when it was happening and was able to log in after server automatically restarted.
When i logged in to troubleshoot he wasn't online.
the server crashed the moment he joined
