When using create 0.5.1.j, the game will crash while loading. I have tested with all three versions available on Modrinth. When using the latest version, the standard mod incompatibility screen comes up saying I need Create 6.0.0 or later. When using the older versions, the game crashes during loading, with an error message saying that I need Create 6.0.0 or later. This conflicts with the description, which says I can use Create 0.5.1 or later. The crash log is attached below.

Create tweaked controllers 1.2.5 and forward is compatible inly with create 6.0 and above, use create tweaked controllers 1.2.4

If you still have a crash when using create tweaked controllers 1.2.4 with create 0.5.1.j, post your crash report

Looking at the crash report, it says I'm using 1.2.5, but Modrinth says I'm using 1.2.4. The crash report was linked in the original issue post.

Edit: I have both restarted Modrinth and used the repair tool to no avail.

Check what's inside your mods folder then, because if the crash report mentions that you have 1.2.5 then you definitely have 1.2.5

It says 1.2.4 in the mods folder, as well as in the mods.toml file. I have also updated forge. Essentially, the crash report is referencing a file that doesn't exist.

You might be looking at the wrong folder or game instance then, the crash report clearly mentions a file named "create_tweaked_controllers-1.20.1-1.2.5.jar" located in "C:/Users/jbear/AppData/Roaming/ModrinthApp/profiles/createMdrt/mods/"
If the file does indeed not exist, then you might have a problem with your launcher
In any case this is not a tweaked controllers issue

I'm having this problem in other places as well. It's definitely on my side. Still very strange, though.
