Replication steps:

1. Hold alt to open Effortless Building radial menu. Select any mode other than default.
2. Shift right click on any Create machine using the Create wrench
3. Machine drops in item form, but the block remains, duplicating the machine

Actually, Anything that prevents the block from being broken will reproduce the bug.
Include FTBChunk and any other protection plugin/mod.

Why not check to see if the original object still exists before making item drop?

This bug should be given enough attention, because most servers have a situation to prevent the block from being destroyed.
