It seems that if you die in crushing wheels while on a server, you get kicked for flying because you're off the ground for too long (assuming the server admin hasn't turned off these checks).

To reproduce this, armor may be necessary to extend the time that the crushing wheel takes to kill the player. I know I was wearing enchanted diamond armor when it happened.

Update: standing in the middle of the crushing wheels (where the controller appears) also seems to get you kicked, even if the crushing wheels are stopped.
