Describe the Bug

When I opened a vault and tried to enter the vault nothing happened and when I died in the overworld/nether and spawned the end screen of the vault had appeared although I wasn't in one. I tried to restart the game, Forge, the mods and break and reopen a vault and nothing fixed it.

Reproduction Steps

1. open the game
2. do something that will bug the vault somehow( I don't know what I did that caused it)

Expected Result

entering a vault and playing without problem
