Create mechanisms constantly break and create ghost blocks.

Description
This has been happening a lot on my server lately. I'll break a machine and it will still appear to be there until I right click. Additionally, blocks seem to constantly disconnect and stop passing rotation through. Weirdly it seems to happen most often at my workshop and only occasionally at my manufactory.
