I have tested this first on a forge server with the same mods as I had. It connected joined and then it disconnected me with the error "Incomplete list of tags received from server."
I tested on a vanilla server. Same error, "Incomplete list of tags received from server"
I uninstalled all of my except create and tried to join
FML and vanilla had the exact same error
After uninstalling create the bug was resovled
I tested a couple of other forge versions with the mod. Same issue
The server console provided no explanation for the issue. Saying I joined and then disconnected

Game Version: 1.16.4
Mod Version: create-mc1.16.3_v0.3 (Curseforge has it listed as a 1.16.4 mod + it loaded with no errors)
Forge version: 1.16.4-forge-35.1.36

Make sure to launch your forge server by running the correct jar file.
Your server will only be able to host mods when launching forge-x.jar as opposed to minecraft_server.jar
