Cant use the mod while Optifine is installed. I checked the git sources page and saw that a lot of people are having the same issue. The players that have Optifine installed cant just join my forge server. All the errors are ticking entity related. Was anyone able to solve the issue without uninstalling optifine?

Try updating optifine to the latest verion (G6)

I can confirm that the latest version of Optifine doesn't (fully?) remedy this. I'm on 1.16.5, Forge 36.0.01, and preview_OptiFine_1.16.5_HD_U_G7_pre2 with players reporting random crashes seemingly specific to chunks with Create machinery in them. I'd rather wait for an update/fix than revert the clients based on the ominous warning from Mojang about 1.16.4 insecurity.

I can play Create normally with Optifine G5, so mabye G6+ is causing issues?
