I believe if you don't connect a shaft to a block that accepts a shaft, when you try to load that chunk again, the game crashes, and you are unable to load that chunk until you remove the mod

It works like Arcafanetiz said or instead of reverting I upgraded to forge 1.16.5

Minecraft 1.16.4 - Forge 35.1.37 when used with optifine, causes the issue
Revert to Forge 35.1.36

Just don't use the latest forge and pre versions of optifine, and you should be fine.
