Same thing happened to me. Wearing full suit of Draconic Armor (Chest piece had Shield Regen and Shield Capacity upgrades up to Draconic level, no other items had upgrades) and using the Sacrificial Dagger from Blood Magic (version 1.10.2-2.1.11-80). I killed myself pulling blood into my altar by self sacrifice. When I re-spawned I received no grave note or grave stone with my items in it. Keep inventory is set to false.
