I was approaching a dead body of a player(player X) that was no longer logged in to the server. A message said "player X has fallen out of the world." What I think could be happening is that the entity is loaded and the chunk hasn't. When this happens the body still has physics and falls through where the chunk is supposed to be. The fix might be to disable physics when the body is in an unloaded chunk.
