Tangentally this happened server side while using War Dance. An apotheosis boss had spawned and I used a skill called "Tackle" to dash into the enemy. Tackle is meant to make you invulnerable for a short amount of time (so you can bump into the foe freely and not get punished for trying to be cool. as all dashing in all mods should do. ;) )
However the boss which I think was a stray fired an arrow just as I bumped into him. it immediately froze my game when I heard the sound fire and lagged the server enough that it caused the watchdog to close it down. My game stayed permanently frozen and my friend was kicked off once it lagged out.

Not sure what caused it or if it'll just actually happen the next time I get shot. But thats what occurred.
