Just like that, it happens.

For more information, i use Elysium Days 4.0.0 Modpack

it doesnt happen in vanilla

I noticed this sometimes happens on some servers that let you punch blocks but they don't break, such as on pvp legacy. I don't fully know how to recreate it though, but it happened once or twice on there.
