First start with First Aid and scaleMaxHealth=true with maxHealth

Everything that was left should be fixed by 8c5ee77
I fixed the desync with dedicated server with 594726e, but the current health = max health on start is a little bit more tricky, and I don't have time for that atm. I'll leave it open though
Seems to be another issue - In multiplayer, your health will be always at maximal, but actually it will be much lower.
So, after you died in multiplayer, your health will be maxed out when you spawn near your bed, but your actual health (in First Aid) will be only 20.
Manually editing health to be 20 after join produced weird effects - your health jumps from 20 to actual value. And moreover - this doesn't happen in singleplayer.
Is there something can be done about this?
This looks like #38 but a bit different - on join random parts will have different health (compare two first pictures).
Head, for example, is fully healthy, but HUD does not display so.
