Reproduction steps
- Build a launchpad structure;
- Place a rocket on it;
- Add fuel and launch;
- When the rocket is mid-air, press space for any times, during which you can hear the sound of picking up items;
- When you reach the destination, there's a high chance you cannot find the launchpads anywhere (it happens to me every time when I travel back to earth from moon).
Base on the info you gave, if you just add fuel to the rocket then take off, your launchpad is going to go with you. When you land on any other planet, you will find your rocket and pad inside the lander you arrive on. If coming back to the Overworld, it will be in the para-chest. If you don't want the launch pad to go with you, place a Launch Controller next to the pad.

I realize that they are supposed to go with me, either in the lander or the para-chest. But in my case, sometimes they just aren't there.
