I was in the End, trying desperately to latch onto something, but died as the hook was shooting out. However, just before I actually died, it kicked me out of the world to the multiplayer select screen (?). I am now unable to rejoin the world, as this happens every time.

This has just occured to me as well. So it isn't an isolated issue.
