[1.12.2] Intermittent crash when destroying blocks with graves

Issue: I am getting an intermittent crash upon destroying blocks that have graves on them. The items get retrieved from the block, however the crash occurs before my character has time to pick them up to inventory.
This just kicks me out of the server, I can rejoin normally.
How to reproduce: Get (un)lucky while retrieving your items from a grave. The crash seems to occur randomly :(
Mod version: GraveStone-1.12-Graves-1.0.14.jar
(I'm also using GraveStone-1.12.2-Extended-1.3.0b6.jar)
Forge version: forge-1.12.2-14.23.2.2654-universal.jar
This crash also happened with version 2641. (I updated to 2654 to be able to use the latest Foamfix.)

Full client crash log here with list of all mods etc. as requested.
There is no corresponding crash on the server. I'm playing on my own private server on a different PC, nothing shows up in the logs.
