I am in the process of testing of creating a kitchen sink modpack to play personally with my mates, It is very long but it is nominally included in the crashlog so I guess just look at it. Obviously this is just an excuse as I am lazy to type the individual names of more than 100 mods.
Additional Information
I am using not enough crashes and it pointed that the crash was generated by this mod.
I played recently, it made another crash, same everything but here is the crashlog for the latest crash if it helps:

Im not sure if it is the exact same crash but the error and stackstrace are very similar.
For me, the game crashes everytime i go through a netherportal from the nether to the overworld, heres the crash file
