Whole stack of blocks in hand for place block quests vanish when trying to place in world.

I have isolated this issue to HQM as it doesn't occur when I remove the mod. I have a set of quests for placing blocks using ore dict. When attempting to place those blocks, the whole stack in hand will vanish and nothing will be placed in world.

Can you provide more information, oike modpack, minecraft and forge version?
