I'm playing 1.12.2 modpack currently (also on 1.16.5) and the ST suddenly broke. It was working, and then when I renamed a mob with a nametag and used it again, it stopped working. It would add a space to the entity's name and the "Silenced" part was gone. Turning off the naming part didn't help either. It would say in the chat that the mob was silenced and then immediately unsilenced.
Uninstalling the mod did nothing. Playing on a different preexisting world did nothing either. :(
I have about 150 mods going. Have been using the mod for a while with no issue.
