I tried my hardest to find any logging of the events, but this one may not be a log-based situation.

Description:
The wither builder as a whole does work fine for what could be stated as 90/95% of the time, however, there seems to be something that is happening "At random" or at least visually at random in which the wither builder will not place Wither skulls but will place the wither sand.
I am having a hard time finding what triggers this as it's happening at unpredictable moments, and I have not dug super deep into any applicable code to attempt to debug it further to help y'all.

All I know is that this situation occurs spontaneously, and can happen anywhere, it does not matter if it's a new chunk, old chunk, a claim, not a claim, a claim that is chunk loaded, etc.

I also had mob griefing on the whole time, so the little fella had the potential to spawn and tear up the world if it wished, but I did try to flip mob griefing on and off again.

I know I have read a lot of history of this issue popping in and out where some people have it and it goes away and some have it and just say "to heck with it" and do it the old way, but I want to get some traction going with this and see if there might be something further that can be done with getting the machine to be a bit more stable or simply having this happen much less.

Just an update, the Wither builder seems to be working and not working, what I mean by that is that I log off from it not working and then I log in and it still does not work but every once and a while I will check it and all the sudden I see it apparently harvested 20/30/60+ nether stars and used up the soul sand and wither skulls even know I never saw ONE spawn ever.

I don't know how that is possible but that's exactly what is happening.
