I have added a screenshot showing what the wither builder created.

Industrial Foregoing Version: 1.12.6
TeslaCoreLib Version: 1.12.2-1.0.15

Using RFTools network to provide materials, (Soulsand and Wither Skeleton Skulls)

The wither builder created the soulsand "T" correctly, but placed 2 wither skeleton skulls and one Regular gray coloured Skeleton skull. This of course would not transform into a Wither.

Breaking the Regular skeleton skull, would consume another Wither Skeleton skull from storage, but the builder would place it on the now empty spot as a regular skull again. I broke the soul sand "T" and the wither builder created a new "T" with Three Standard skeleton skulls, and consumed 3 more Wither skeleton skulls from inventory.

Occasionally, the wither builder does place three wither skeleton skulls, and a Wither is generated.

I'm finding that I'm losing about 50% of my Wither Skeleton Skulls to this issue. As they're not exactly easy to come by, this is a major issue for me.

After a server restart, I'm still having the same issues.

I turned off the Wither Builder, and manually added the third Wither skull.

This is the Wither builder before turning it back on... The second photo is what the wither builder created. This time all three Wither skulls were placed as Regular skeleton skulls by the Wither Builder.

I just discovered this morning, that the wither builder was building straddling a chunk border. I don't know if this is relevant, but thought it might be.

After moving the killing chamber so that everything was inside the chunk, it ran perfectly all day. Until about 5 minutes ago when it started doing the same thing again.

It appears that straddling a chunk border is not a cause of this issue.

Breaking and replacing wither builder fixes the issue. It seems to be hidden feature to avoid withers autobuilt by players who leave it unattended for a long time.

Still an issue, playing Enigmatica 2 Expert 1.66c, IF version 1.12.2-1.12.12-236.
