What happened?
Shaders are bugged.

Entities have no color, Player model has no color, items also have no color, some black squares appear randomly on screen, and items are inside gray-black squares, also this happens with all shaders.
Mod list:
The problem also happens with Iris & Sodium isolated.

This is likely unfixable in Iris / the shader. The graphics drivers for HD 4000 on Windows are very broken unfortunately because they have not been actively developed for a long time.
However, the drivers for HD 4000 on Linux are actively developed and maintained and they work much better. Realistically if you want working shaders with your existing hardware, you might not be able to get that on Windows.
