I have had this issue multiple times. The most recent occurred when I was placing an item from my hotbar into my backpack. I was holding down shift and mis-clicked the mouse wheel. The item I was trying to move disappeared as well as my backpack and all items in it.

Lost 2 backpacks due to this earlier today, running Direwolf20 1.12

Would one of you mind providing either pictures, a gif, of a detailed set of step-by-step instructions to replicate this? It would help a lot, as I can't quite figure out how to do it myself with inventory sorter.

CC'ing @cpw - this might be on your end/require special handling, not sure yet.

Definitely happens on Direwolf20 1.12 for some reason. One of my users just reported losing a backpack mysteriously and this appears to be the most likely cause. I have yet to see it firsthand on account of not using backpacks myself and not going out of my way to test it but... there is a there there.
Any chance you guys are running Spongeforge when this is happening? Disregard this. I forgot to install sponge so it's irrelevant

Also, CPW probably isn't the right person to cc about Minecraft/forge issues at this point, on account of him quitting after Microsoft acquired MC. I'm not sure who is but unless something has changed, it's not him anymore
