[1.12.2] Server Crash

Got this crash with this build:
LeatherWorks-1.12-1.75.0.32.jar

Forge:
forge-1.12.2-14.23.3.2655-universal.jar

Player was fighting a witch and then died and the server shut down immediately.

Getting the same error after getting kill by a fire charge attack from the magma monsters mod.

It keeps happening every time after any death.
