Dupe Glitch and Console Error

I believe a user posted about this a while ago but here is more information on it.
Solution found was to manually respawn each existing persistent mob that had equipped items, which seems to stop the duplication chunk error. We did this on a Paper 1.21.4 server (derex smp).
