Shortcut keys sometimes fail

My first guess is that you have some keys getting stuck on, such as Alt or Shift or maybe Mouse4. Most of the hotkeys are by default "strict" ie. set to not allow any extra keys to be pressed.
There is a keybind debug option in malilib that you can use to see which keys it currently sees as being pressed. You can access the malilib config menu either via Mod Menu on the Fabric versions, or via A + C. If you want to use that hotkey, then you would probably want to make that allow extra keys before the issue happens next and you would want to open the GUI to enable the debug option... The "Advanced Keybind Settings" widget next to the hotkey config button can be used to change that option.
