I play with fabric, Patchouli is not out, for Fabric at 1.21.4 in the crash log:
[23:46:41] [main/WARN]: Warnings were found!

• Mod 'Living Things' (livingthings) 1.21.4-2.2.0 recommends any version of patchouli, which is missing!
• You should install any version of patchouli for the optimal experience.

That should definitely not be the reason for a crash. Patchouli is listed as recommended by this mod, that means if it is not available everything still works fine.
