Upgrades disappear arbitrarily when placed in the gui slot

At times speed and energy upgrades disappear completely when placed in the upgrades gui slot in machines. Seems to affect most mekanism machines.
Both survival built and spawned upgrades have a chance to disappear when placed in slot one by one or by stack of any number. Placing upgrades again after the first one disappears usually works, sometimes even those disappear.
A rather baffling bug, does not generate any log entries.
