Steps to reproduce the issue / details

I'm not sure how it happens, I wasn't using Minecolonies actively (changing anything) or in Twilight Forest (but the dimension is activated).

I received the following error attached that seem to related Minecolonies citizens with Twilight Forest. Possibly a citizen somehow falling through the portal and causing confusion while diseased?
