Reproduction steps:

1. Put down an MI barrel
2. Attach a Create packager to the barrel
3. Insert items into barrel

Are you able to reproduce the crash in a fresh instance with only MI and Create mods (plus dependencies and NeoForge) installed?

No, it seems to work fine there. Still investigating which mod causes it.
