If I have a shield (or other usable item) in my off-hand, I cannot both aim down sights and fire at the same time. This doesn't seem to be an issue with items that can't be used in some way: eg. leather or bullets. However, shields prevent aim-firing as long as they're equipped, and food prevents it if you can eat at the moment.
