It appears that the planes biome type couldn't be found. What version of forge/Minecraft are you running?

1.12.2 minecraft and forge-1.12.2-14.23.4.2759-universal

That's why it's not working, you're using a 1.12 mod in the 1.12.2 client. Using the 1.12 client will fix this.
