Game crush on break block

Version 1.21.1, Neoforge
Random crash in DivineRPG. Before every crush I attempt to break some block (this time a vanilla netherrack) with Vengeance Pickaxe from EvilCraft, but not every break cause crash, the chance is rare like 1 : 500.
Vengeance Pickaxe during use causes spawn of invisible aggro mobs which can attack player, but disappears after short time. Looks like the crash caused by such monster AI action and call some callback in DivineRPG.
Also sometimes such invisible "ghost" mobs are based on existing mobs, may be this mob was based on some DivineRPG mob in the Nether.
